1.推薦函 Recommendation Letter

(完成申請程序，請於系統下載，並請指導教師簽名後，繳交系辦)。

The applicant can download this Recommendation Letter after the on-line application is completed. Please note that this letter has to be signed by the advisor(s) and returned to the Group Office.

2.「學術研究倫理教育課程」修課證明書Certificate for completion of the Research Ethics Education Online Program (for applicants admitted into the Graduate School after June, 2016) (105 學年度起，入學之學生需繳交)

碩士班口試委員：3~5 人。

The number of the Thesis Oral Examination (Master Degree) committee members should be ranged from 3 to 5.

博士班口試委員：5~9 人。

The number of the Dissertation Oral Examination (PhD degree) committee members should be ranged from 5 to 9.

口試委員 1/3 以上需為校外人士。

At least one-third of the committee members are external members.

口試委員請先徵求指導教師同意。

Committees’ designation has to be consented by advisor(s).

口試費(含車馬費)核發標準： 碩士生每生上限 4000 元。 博士生每生上限 15000 元。

Guideline for remuneration (including travelling expenses): Limited to NT 4,000 for Thesis Oral Examination

Limited to NT 15,000 for Dissertation Oral Examination

• 確定口試時間，請至系辦登記口試費，口試費以銀行轉帳方式入申請人遠銀帳戶。
• Register your account of Far Eastern International Bank for the transfer of Examination Committee’s remuneration.
• 口試時，應給口試委員以下資料：
• (1)評分表 (請至「學位考試系統」下載)
• (2)口試費收據 (請至會計室網頁下載)
• The committee members should have following materials during the exam:

Evaluation Form (download from applicant’s Portal)

Receipt Form (download from the website of accounting office)

推薦函、審定書及碩/博士論文之論文題目，需完全一樣，敬請特別注意。

Ensure the Thesis/Dissertation title is consistent with the one appeared on the Recommendation Letter and Verification letter from the Oral Examination Committee.

口試結束後，請繳回收據及評分表至系辦

畢業論文繳交

1.論文2本

Two hard copies of the Thesis/Dissertation.

2.審定書正本1份

1 original copy of the Verification Letter from the Oral Examination Committee.

3.元智大學 電子論文上網授權書，正本1份、影印本2份

One hard copy and two photocopies of Yuan Ze University Authorization for Public Access of Thesis/Dissertation.

4.國家圖書館電子論文上網授權書， 正本1份、影印本2份

One hard copy and two photocopies of National Central Library Authorization for Public Access of Thesis/Dissertation.

5.若紙本論文有申請延後公開(僅3理由才能延後)，則需要繳交「國家圖書館學位論文延後公開申請書」，正本1份、影本2份。

One hard copy and two photocopies of Application for Embargo of Thesis/ Dissertation if you request a delay in publication of your thesis/dissertation.

(1) 逾期繳交視同下學期畢業。

Overdue submission will result in one-semester delay of graduation.

(2) 授權書之簽名，請務必使用藍色原子筆。

Notices

1.博士班學生，需先申請填寫「博士畢業口試資格考審查表」(並檢附相關資料)，並先上線申請論文口試，待會議確定畢業資格後，再進行畢業口試﹔若沒通過畢業口試資格，則需上線申請撤銷口試。

「博士班畢業口試資格考審查表」申請截止日：2021.10.29。

PhD students who apply for the Dissertation Oral Exam should first verify his/her eligibility for the degree by completion of “Application form for Doctoral Qualifying Exam”, which is due on Oct. 29, 2021. Detailed regulations can be found in Regulations Governing the Studies of Doctoral Program.

2.若口試委員為碩士級稀少性學科專業人士時，請事先提出申請，以便安排會議審查，逾時恕不受理。(截止收件日：2021.10.15)

The committee designation has to be consented by the Group Faculty meeting if the applicant would like to designate a professional with a Master degree in scarce fields as the committee member. In this regard, the application should visit the Group Office prior to Oct. 15, 2021 to apply for the designation

若無法如期順利於本學期完成口試時，請線上申請撤銷。(申請截止日：2022.01.21)

Please withdraw the application online in case the oral exam will not be completed in the applied semester. (Before Jan 21, 2022)
